JD, 77-Year-Old Caucasian Female on Hemodialysis

CLINICAL INFORMATION:

History of Current Illness:

• 77-year-old Caucasian female
• CKD etiology unknown
• HD thrice weekly since November 2005
• Not eligible for transplant

Past Medical and Surgical History:

• Ileostomy, long term; well adjusted to limitations/diet modification
• Hypertension, controlled with antihypertensives
• No other comorbidities reported

Social History:

• Widowed, lives alone
• Drives self to dialysis
• Two adult children who live locally

Family History:

• No known CKD in family
• HTN, mother and maternal grandmother
