Case Title:
40-Year-Old Caucasian Male With Living-Related Kidney Transplant

CLINICAL INFORMATION:

History of Present Illness (April 2007):

  • 40-year-old Caucasian male with living-related kidney transplant April 2006 from his brother
  • CKD secondary to chronic glomerulonephritis
  • Excellent allograft function since April 2006
  • 34 pound (15%) weight gain since transplant
  • New onset diabetes after transplant (NODAT) with good glycemic control at this time. Has not required insulin since immediate post-transplant period; no oral medication for glucose control. Checks CBGs sporadically at this point

Past Medical and Surgical History:

  • First kidney transplant (deceased donor transplant) received in 1984; failed in 1999 due to chronic rejection
  • Returned to hemodialysis while awaiting second transplant
  • Multiple squamous cell and basal cell carcinoma on both hands. Followed closely by dermatologist
  • Hypertension under excellent control with triple therapy
  • GERD stable on omeprazole

Medications:

  • tacrolimus 2 mg bid
  • mycophenolate mofetil 500 mg tid
  • prednisone 10 mg qd
  • omeprazole 20 mg qd
  • metoprolol 125 mg bid
  • candesartan cilexetil 16 mg qd
  • nifedipine XL 9 mg qd
  • calcium carbonate 600 mg tid
  • MVI bid
  • folic acid 5 mg qd
