History of Present Illness
- 56-Year-Old Hispanic female with CKD secondary to diabetes mellitus Type II
- Started hemodialysis 2 weeks ago
- Has a basilic vein transposition AVF
Past Medical History
- Diabetes mellitus clinically recognized for 7 years, treated with oral hypoglycemic agents and insulin
- Hypertension for 10 years, well controlled
Medications
- Lisinopril 20 mg q day
- Amlodipine 5 mg q day
- Insulin 70/30 35 units q AM, 15 units q PM
- Pioglitazone 15 mg q day
- Temazepam 15 mg q hs PRN
- Ranitidine 20 mg q hs
- Simvastatin 20 mg q day
- Finished course of ergocalciferol 50,000 IU q month x 6 months
