CLINICAL INFORMATION:

History of Current Illness:

• 37-year-old Caucasian Male
• Type 1 diabetes mellitus
• Proteinuria
• Hypertension

Past Medical and Surgical History:

• Diabetes mellitus since age 18
• Insulin pump instituted 5 years ago
• Urinary protein (1 gm/24 hr) detected 1 year ago
• Started on ACE inhibitor and ARB
• Urinary protein decreased to 500 mg
• Known retinopathy
• Laser treatments
• Hypertension well controlled
• On loop diuretic

Social History:

• Supportive spouse
• 2 teenage children
• Accountant
• Works long, erratic hours
• Rare exercise

Family History:

• Strong history of diabetes mellitus
• Mother died with diabetes mellitus
• Father with known hypertension
